What should I bring?
Please bring your Photo ID and your insurance card to your first appointment. To save time, please fill out your new patient forms online. If you don’t have time to complete them prior to your appointment, please come 10 – 15 min early to fill out the paperwork. Any cash payments, co-pays or coinsurance due will be collected at each appointment. We accept cash, check, Health Savings Cards (Flex Account) cards, Visa, Master Card and American Express.

What should I wear?
Please wear comfortable, loose-fitting clothing on your first visit or bring them along to change into while you are here.

How long will my appointment be?
Your initial visit/evaluation will be a bit longer than most of your appointments going forward. Plan for a little over 1 hr of time.

What insurances do you take?
We accept all insurance carriers.  We are in network with Aetna, BCBS, United and Cigna. Please see our insurance section for more details.

Do you offer cash discounts?
Yes. Our cash pay (or self pay) rate is $150 for your 1st visit (60 min) then $100/visit for each additional 45 min session.

How many sessions will I need?
You and your therapist will come up with a customized plan for a successful recovery. Your rehab program will be tailored specifically to you, your recovery requirements and to what your schedule and budget will allow for.

Do I need a referral?
The state of Texas allows your physical therapist to do an evaluation without a referral. If any treatment or sessions are needed after the evaluation, a referral or prescription will be needed from your doctor, chiropractor or nurse practioner. Some physicians are happy to fax a referral if you have already been seen for the condition in which you are needing therapy.  PT referrals can be faxed to (832) 384-9106.
